Drivers/Linux: make host kernel modules use make file templates.
bugref:4567: Linux kernel driver maintenance

Rework the make files from the host kernel drivers to use the
Makefile.include.header and Makefile.include.footer templates, and clean up
some obsolete scripting in the make files.
