VirtualBox needs you to add a dummy "everything" filter before adding other filters for USB devices. It should add the everything filter by default and mask off all USB devices not listed in the USB device list. The dummy filter is not intuitive...
Win98 guest running on WinXP host if that matters.

As I understand how the USB support works, you have to go to the menu of the Guest window, and then indicate which USB device you want to hook in. The idea is not to add all USB devices *by default*.
The complaint of this bug is that the way to enable USB support is not-intuitive (look in the forums for the error string to see how many people have a hard time setting up USB support and think it's broken in VirtualBox). If you do not add the "everything" filter, VirtualBox's USB support seems broken (i.e., all devices seem to be always busy).

Actually this bug is obsolete since ages. There is no empty USB filter required anymore.

Looks like this bug is back again.
Version 5.2.14 r123301 (Qt5.6.3) on MacOS High Sierra. No USB devices are shown until an "All" filter is created.
I don't agree as USB devices which are made available for guests are not available for the host anymore and this is often not the desired behavior.
