my version : VirtualBox Graphical User Interface Version 5.1.2 r108956
failure: Failed to install the Extension Pack D:/Users/Yaniv/Downloads/Oracle_VM_VirtualBox_Extension_Pack-5.1.2-108956.vbox-extpack.

Sounds like a duplicate of #7972. Try the suggestion in the most recent comment (Disabled McAfee Real Time Scanning and was able to install the ext pack successfully).
